Submission Information

How to Apply

  1. Determine eligibility.
  2. Complete the application.  The application must be typed. Provide all requested support material.
  3. Completed applications must be submitted by mail (postmarked, not metered) or hand delivered by 5:00PM of the deadline dateLate applications will not be considered.

Work Samples

Submitting Work Samples
Applicants may submit samples of work using any one of the following formats:

  • Videotape (one)
  • Audio Cassette Tape (one)
  • Manuscript (10 pages maximum, double spaced)
  • Slides (8 maximum)
  • CDs or cassette tapes
  • Photographs

Videotape

  1. Must be in VHS format;
  2. Label cassette and box with your name(s), title of tape, date of completion, length, black-and-white or color, and credits specifying your role;
  3. Queue tape to the point that you wish the Panel to view.

Manuscripts

  1. Each manuscript should include a title page stating your name, address and year work was written;
  2. Manuscripts must be in English and typed double-spaced on 8x11 paper.  Submit no more than 10 pages.

Slides

  1. Submit a maximum of eight (8) 35 mm slides;
  2. Mark your slides as follows:
    1. Your name;
    2. Number slides from 1 – 8;
    3. Place a red dot on the lower left corner of each slide.
  3. Enclose the slides in a clear plastic sheet.  (NOTE: Glass slides will not be accepted.  Do not use tape or adhesive labels.)

CDs/Cassette Tapes

  1. Please label all CDs and cassettes;
  2. For CDs, identify the track(s) -- no more than 3 -- panelists should hear;
  3. For cassettes, please queue tape to the music panelists should hear.
